St. Mary's Community Care Professionals provides a comprehensive spectrum
of skilled and supportive services directly and through collaborative agreements.
Our nurses, therapists, and other caregivers are experienced in family-centered
care for infants, children, adolescents, and adults. As part of St. Mary's Healthcare
System for Children, we share a 127 year old tradition of excellence in providing
complex medical care and intensive rehabilitation to children and adolescents—a
tradition that St. Mary's Community Care Professionals now proudly offers to adult
populations as well.
Our Patients...
Patients are referred to us from health systems, hospitals, clinics, home care
agencies, treatment programs, and schools. They range from newborn infants to
elderly patients challenged by disabling conditions. In addition to caring for
patients with episodic or chronic illnesses who need specialized nursing care
and comprehensive rehabilitation therapies, we also serve developmentally disabled
children, adolescents, and adults. We can serve patients who reside in the five
boroughs of New York City or in Nassau County with medical conditions that include

Our Programs...
Pediatric Care provided by caregivers who are
skilled in caring for infants, children, and adolescents. Focused on the family
as a primary care partner, our program offers social work services, supportive
care, and rehabilitation in addition to meeting a full range of medical needs.
Maternal/Child program gives newborn infants
and their mothers a full spectrum of nursing and supportive services, including
prenatal care, postnatal, and well baby care, lactation counseling, care for complications
of prematurity, and parent education.
Developmentally Disabled adolescents and adults
can receive specialized psychosocial and educational services integrated with
the treatment of specific medical conditions.
Geriatric Care focuses on allowing our elder
adults to remain in their homes in a safe and supportive environment. In addition
to a full range of nursing and rehabilitative services, we provide support for
independent living and maintenance of a safe home environment.
Adult Episodic Acute Illness is treated with
a broad spectrum of services for those recovering from illness, surgery, or trauma.
In addition to nursing and rehabilitative care, we provide supportive services
such as homemaking, housekeeping, and personal care.
AIDS patients of all ages receive comprehensive
nursing and rehabilitative care, psychosocial and supportive services from caregivers
trained to provide episodic acute and chronic care including infusion therapy.
High Tech Care such as infusion therapy, peritoneal
dialysis, ventilator care, total parenteral nutrition, pulse oximetry, and patient
controlled analgesia is provided by skilled nursed and therapists.
Personnel Staffing is offered to organizations in need of experienced staff in
healthcare, educational, and residential settings.
