St. Mary's Healthcare System for Children is the largest provider of long-term
home healthcare for children in New York State. In the more than 20 years since
the home healthcare program was pioneered, St. Mary's has served over 10,000 children
and has demonstrated the ability to provide multidisciplinary services to the
most clinically complex child.
Our skilled professionals promote a family-centered philosophy of care complete
with bilingual support. We work with both the family and the physician to develop
and implement a customized care plan that will optimize the effectiveness of every
treatment and intervention to achieve the desired clinical outcomes.
Programs and Services We Offer
Depending on the acuity of the child's condition, family support, home environment, and payor
source, the following home healthcare options are available:
LOMBARDI PROGRAM
Long-Term Home Healthcare Program
This program is unique in its wide array of services and intensive case management
for children with a variety of chronic, complex medical problems who require home
healthcare services over an extended period of time.
Additional programs include:
• AIDS Home Care Program
• AIDS/HIV Mental Health Program
- Available for HIV-positive patients and their families, funded by Title I of
the Ryan White C.A.R.E. Act
• Vehicular Access to Neighborhoods (VAN) Program
- Multidisciplinary teams provide care to children and families residing in difficult
to serve areas
ST. MARY'S METROPOLITAN HOME CARE FOR KIDS
Certified Home Health Agency
St. Mary's has partnered with the Metropolitan Jewish Healthcare System to
provide maternal child health and pediatric home healthcare services to children
and families who require short-term home healthcare.
ST. MARY'S COMMUNITY CARE PROFESSIONALS
Licensed Home Care Service Agency
St. Mary's licensed agency is designed to provide specialty pediatric rehabilitation
and nursing services to children, families, and other home care agencies and institutions.
CARE AT HOME PROGRAM
St. Mary's pediatric nurses and professional staff provide comprehensive
case management for the delivery of multidisciplinary services to children and
families.
Service Areas: Depending on the program
selected, services are available in the five boroughs of New York City, Nassau,
Suffolk, Westchester and Rockland counties. St. Mary's Community Care Professionals
also serve Orange, Dutchess, Putnam, Sullivan, and Ulster counties.
