We understand that children and young adults with neuromuscular diseases often need ongoing support to help reduce and control their symptoms which may include: muscle loss/weakness, movement & balance issues, numbness, tingling, trouble swallowing and trouble breathing. St. Mary’s Neuromuscular Home Management program is designed to provide comprehensive education and ongoing support to children and young adults diagnosed with neuromuscular diseases such as: Spinal Muscular Atrophy (SMA), Muscular Dystrophy, Peripheral Neuropathy, Cerebral Palsy, and other movement disorders. Our goal is to help patients and their families achieve better control of their disease through education and links to additional resources and networks of other families coping with a neuromuscular diagnosis. St. Mary’s Home Care employs a team of skilled nurses, social workers, nutritionists and therapists who work together to provide patient/family centered care and education in the home while collaborating with the patient’s physician to assist patients and their families to understand the disease diagnosis, follow their treatment plans and manage their symptoms effectively at home.

Review of a patient’s care plan, medications and potential side effects.
A thorough assessment of the home is performed to identify and reduce potential fall/barrier hazards.
An assessment of the patient’s rehabilitation needs and activities is conducted. Therapy visits are ordered as needed and patients and families are educated on how to make helpful changes & maintain body function and therapies.
Patients receive automated messages and remote monitoring which allow our Telehealth RN to triage patient alerts to the home care nurse who will make a home visit if necessary.
Patients and families are educated on how to recognize the signs and symptoms of distress that require emergency treatment.
