St. Mary’s is currently seeking a full time Physical Therapist to join their home care team. The qualified candidate will provide home based services for treatment of patients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care. Evaluates, and regularly re-evaluates the rehabilitation needs of the patient; initiates, develops, implements and makes necessary revisions of the patient’s plan of care. In addition, the Therapist will assist with the coordination of the total plan of care and maintains continuity of patient care by liaising with other health professionals assigned to the same patients.

Position Requirements

  • Licensed in the state as a physical therapist. If licenses are registered prior to 1966, meets the alternative qualifications cited in the Medicare Conditions of Participation, Section 405.12 (1) (2) (3)
  • Minimum of one year of experience in pediatric physical therapy and/or pediatric affiliation.
  • Must be able to travel to conduct home based services.
  • Must have reliable transportation and valid drivers license.
