When specialized care is needed at home, St. Mary’s Home Care, a special needs certified home health agency (CHHA), is the source families trust—and with good reason: We
pioneered the first long-term pediatric home healthcare program in New York over 30 years ago.
Today, we remain the largest and most comprehensive provider of services to children with special healthcare needs, delivering care into the homes of thousands of patients who require specialized medical, educational and rehabilitative care. St. Mary’s Home Care provides short-term skilled care for patients from newborn to 44 years of age.

Compassionate, Family-Centered Care
At St. Mary’s home care, we know the vital role loved ones play in a patient’s recovery—and our family-centered team approach makes the most of it. Our compassionate team of clinicians works closely with discharge planners and family members to ensure a smooth transition home. Once there, we coordinate care with the patient’s physician and other health professionals—as well as the family—to make the most of their treatment and minimize the family’s fears and concerns.
St. Mary’s Case Management department helps patients and families navigate the challenges of caring for a loved one at home. Case management assists families in accessing a full range of resources and programs within the community. Skilled case managers coordinate an array of services including securing private duty nursing, attending school meetings, coordinating multiple therapies, working with regulatory agencies, working with contractors on environmental modifications to the home, and providing support to the patient’s family.
Known for our wealth of experience and expertise in Case Management for children, St. Mary’s has enhanced our services to include adults and seniors. St. Mary’s Case Management services are offered through the Care at Home Program, Nursing Homes Transition and Diversion Program, and Community Care Professionals.
