St. Mary’s Staff Participated in NY Presbyterian Panel Discussion

On June 12, 2017 Herminia Alfau, M.S. Ed, Supervisor of Health Home Serving Children and Danielle Toto, LMSW, Supervisor, Social Work, St. Mary’s Home Care,  participated in a panel discussion at New York Presbyterian’s Collaborator Symposium about their experience using Healthify, a new web-based community resource directory.  Herminia and Danielle were chosen to serve as test users earlier this year and their feedback helped NYP and the Healthify team incorporate suggestions to help make the site more robust and user friendly. Healthify’s mission runs parallel to St. Mary’s –  to improve outcomes and lower costs for vulnerable beneficiaries.

The Collaborator Symposium was attended by over 100 partners who are part of New York Presbyterian’s Performing Provider System (PPS). The PPS was formed as part of New York State’s Delivery System Reform Incentive Program (DSRIP) to promote community-level collaborations and system reform to achieve a 25% reduction in avoidable hospital use over five years.
